Medical - Our medical plans are designed to provide employees and their eligible family members with extensive coverage and protection against certain medical expenses. Employees can choose from health maintenance organizations (HMOs) and preferred provider organizations (PPOs).

Dental - LS9's dental plan is designed to help employees pay for the costs of dental care for themselves and their eligible family members. Preventive, basic, and restorative services are covered at varying levels. Participants may take advantage of reduced out-of-pocket costs when they use dentists in a nationwide network of participating providers.

Vision Care - LS9 offers affordable, high-quality eye care coverage and offers access to one of the largest networks nationwide.

Holidays and Vacations - LS9 believes that taking regular vacation is essential to the health and productivity of every employee. Instead of offering separate vacation, sick leave, or personal days, LS9 offers a paid time off ("PTO") program that combines all of these benefits. We believe this program will give employees the flexibility to manage their time off. LS9 offers personal time off beginning at 3 weeks. LS9 will recognize 10 paid holidays in 2008.

Tax-Savings Benefits
LS9's flexible spending accounts allow employees to pay for certain health care and dependent care expenses before federal income taxes and Social Security taxes are withheld, enabling employees to pay less in taxes and have more disposable income.

Health Care Flexible Spending Account - This benefit allows employees to pay for up to $3,000 per year of eligible out-of-pocket health care expenses in tax-free dollars.

Dependent Care Flexible Spending Account - This benefit allows employees to pay for up to $5,000 per year of eligible child and elder care expenses per household in tax-free dollars.

Accident and Life Insurance
LS9 offers life insurance benefits that are designed to help employees provide financial security for their survivors in case of death. All eligible employees of LS9 are eligible for a Life and Accidental Death and Dismemberment (AD&D) benefit of your annual salary to a maximum of $150,000

Short Term and Long Term Income Protection
LS9 offers medical leave programs that are designed to provide continuing income for employees unable to work due to illness, injury, or pregnancy.

Medical Leave - Our medical leave programs provide compensation for eligible employees unable to work due to illness or injury.

Extended Disability Plan - Eligible employees who are scheduled to work at least 30 hours per week may receive an extended salary-based monthly benefit under the LS9 Disability Plan.

Retirement and Investment Savings Plans
LS9 offers a retirement savings 401(k) plan to help employees save for the future.

Retirement Savings Plan - Subject to limits under federal law, the LS9 401(k) plan allows employees to contribute from 1% to 90% of salary on a pre-tax basis - up to a maximum of $15,500 in 2008 - to any combination of multiple investment options. Employees may participate in the plan immediately upon hire.
